1. Overview
THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Healthcare providers and pharmacies that treat you through the Solved platform are required by HIPAA to maintain the privacy of your protected health information ("PHI") and to provide you with this Notice. They will follow the terms currently in effect.
2. How We May Use and Disclose Your PHI
Treatment. Providers may use and share PHI to evaluate, prescribe, and follow up on your treatment, and to coordinate with the dispensing pharmacy.
Payment. PHI may be used to bill you and to obtain payment, including authorizing payment processors.
Healthcare operations. PHI may be used for quality review, training, accreditation, audits, and other operational activities of the providers and pharmacies on the platform.
As required or permitted by law. PHI may be disclosed for public-health activities, to report abuse or neglect, for health oversight, in judicial or administrative proceedings, for law-enforcement purposes, to coroners or funeral directors, for organ donation, for research subject to safeguards, to avert a serious threat to health or safety, for specialized government functions, and for workers' compensation.
4. Your Rights
You have the right to: request access to and a copy of your PHI; request an amendment to your PHI; request an accounting of certain disclosures; request restrictions on certain uses and disclosures (which we are not always required to honor); request confidential communications by alternative means or at alternative locations; and receive a paper copy of this Notice.
5. Breach Notification
You will be notified following a breach of unsecured PHI as required by law.
6. Changes to This Notice
We may change the terms of this Notice at any time. The new Notice will be effective for all PHI we maintain. The current version is always available on this page.
7. Complaints
If you believe your privacy rights have been violated, you may file a complaint with us at privacy@solvedstrips.com or with the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.
